Home Care: Once we have taken your claim application, the first step is for our licensed healthcare provider to develop a care plan especially for you and/or your spouse. Our team’s aggregate experience of over 100 years in the healthcare industry insures that you will receive care quickly (typically within 30 days), not sometime in the far future.
Assisted Living: If care needs become more complex, and can no longer be adequately met at home, then we’ll help you navigate change, transitioning into an Assisted Living community. Our Assisted Living Program provides access to numerous communities that offer warm caring, respectful environments and will support daily care needs.
Care Management: At times, a veteran’s care needs may become more complex and require a higher level of coordination and management. That’s when VeteranCare can match our veteran clients with a Professional Care Manager–typically an experienced Nurse or Social Worker, who can assist them to navigate these challenges.

The Care Manager will conduct a professional assessment, evaluating medical and psycho-social history and current concerns. They will create a Care Plan – a roadmap and action plan to optimize the care of the veteran and/or spouse.
A Care Manager is knowledgeable regarding local resources such as physicians, hospitals, therapists, and rehabilitation facilities. A Professional Care Manager can help the veteran, spouse, and family access all the services necessary to stay at home independently with support, or alternatively help them safely transition to an appropriate Assisted Living Facility, if and when it becomes necessary. A Care Manager serves our veterans and spouses as trusted advisors to optimize their care.
While not everyone qualifies, we are here to help you navigate and will do our best to help you. There will never be any out of pocket expense for you.

We are a veteran care coordination company, assisting veterans and their spouses in enrolling into a federally funded claims program, administered through the Veterans Administration (VA). The VA’s role is to process veteran and spouse claims.
